Oregon State Police (OSP) is continuing the investigation into the cause of Monday morning's fatal traffic crash that occurred on Highway 20 east of Eddyville in Lincoln County. The crash resulted in the death of a Springfield-area man.

According to Lieutenant Justin McGladrey, on April 6, 2015, at approximately 12:07 a.m., a 2000 Dodge Durango sport utility vehicle, driven by IVAN SANDRIA CRUZ, age 24 from Newport, was westbound on Highway 20 near milepost 28. A 1995 Honda Accord station wagon being driven by ANDREW RAYMOND KOLOSKI, age 35, from Springfield, was eastbound at the same location. The cause of the crash is still under investigation however, it is known that the vehicles collided nearly head-on. KOLOSKI was pronounced deceased at the scene.

SANDRIA CRUZ and a juvenile 7-year-old male passenger were transported by ground ambulance to Good Samaritan Hospital in Corvallis where CRUZ is receiving treatment for non-life threatening injures. The juvenile passenger was treated and released from the hospital.

OSP was assisted at the scene by members of the Lincoln County Sheriff's Office, Toledo Fire Department, ODOT, Pac West Ambulance, Lincoln County District Attorney's Office, and Lincoln County Medical Examiner.

Oregon State Police (OSP) troopers are continuing to investigate a fatal traffic crash that occurred shortly before 1:00 a.m., on April 6, 2015, on Highway 20 at MP 27.5 near Eddyville. Highway 20 has been blocked for most of the morning as investigators process the crash scene.

OSP is being assisted by the Oregon Department of Transportation and the Lincoln County District Attorney's office. Additional information will be released when it is appropriate and available.
